  Usually used to provide information such as phone and fax numbers, addresses, email links, and other information such as possibly directions, the "Contact Us" page is a useful way for the user to quickly access various methods to put him/her in touch with your organization.

Alternatively, you can provide a form such as this one below for the user to fill out and send to you.  This may be used to request a brochure, product literature, request a quote, or whatever else you may have a need for.  This is a great way to take advantage of the main advantage the Internet offers--interactivity.  Don't just respond to user form submissions, use them as a tool to find out what they are looking for and how your organization may enable them to meet those needs, or perhaps to provide research data, accept suggestions, ...be creative!  (that's what the Internet is really all about).  As with the rest of the website--the form is completely customizable!

A form example:  Form results can be routed to the email of a person in your organization, an HTML file (to be viewable on-line), a database, or a .txt file.  In addition, upon submitting such a form, the "confirmation" screen that the user will see is also fully customizable
